The New
York Cardiothoracic Surgical Group performs cardiac, thoracic,
and vascular (carotid, aortic aneurysm) surgery at Maimonides
Medical Center. We have a cumulative experience of nearly 22,000
open-heart procedures.
This is an exciting time in cardiac surgery with the
technological advances that are taking place, particularly in
the area of minimally invasive cardiac surgery. Our group is
playing a leading role in the advancement of the technique and
in teaching these procedures to other surgeons.
We presently perform 98% of our coronary artery bypass
operations without the use of the heart lung machine. In
addition we use a large percentage of arterial conduits for
bypass grafting, and when the saphenous vein grafts are needed,
they are procured endoscopically through a very small incision
in the lower thigh. This technique has drastically reduced the
complications associated with saphenous vein removal. For the
patients for whom coronary artery bypass grafting is not
feasible and who are not candidates for PTCA or stent placement,
we perform Transmyocardial Revascularization (TMR) using a
laser.
In the area of valve surgery, we are very active in utilizing
the latest prosthesis that has the greatest benefit for the
patient. The stentless freestyle valve, which was FDA approved
approximately 2 years ago (Dr. Jacobowitz was a participant in
the clinical trials) has superb hemodynamics with essentially a
zero trans-valvular gradient. Studies have demonstrated
regression of ventricular hypertrophy, and have been found to be
advantageous for patients with decreased left ventricular
function. Homograft aortic valves and the Ross procedure are
also performed by our group in those patients who would most
benefit. We have a large experience with Mitral Valve Repair and
frequently approach our valve surgery through a minimally
invasive approach, which has been shown to decrease
postoperative pain and greatly enhance the cosmetic result.
In the area of vascular surgery, we have the largest published
series of combined coronary artery bypass grafting and abdominal
aortic aneurysmectomy with zero mortality in a consecutive
series of 21 patients. We have a very large experience in
combined carotid endarterectomy and heart surgery, again with
excellent results and without any increased risk as compared to
the isolated procedures. Type I & II aortic dissections and
aortic aneurysm, both acute and chronic, have been performed by
all members of our group. We are available to handle such
emergencies and can arrange for rapid transfer to our
institution.
